Activity Matrix Form for Switch Use/Data collection

I have attached an Activity Matrix form that SETC has used for years to help in planning implementation of both AAC and switch use. There is a Sample Activity Matrix and a Blank Activity Matrix attached.  This could also be edited to include a column for data collection or at least to track follow-through of the plan.
